Glaucoma: The Eye Disease That Steals Your Sight Before You Notice It's Gone

Imagine losing your peripheral vision so gradually that you never register the change — until the damage is irreversible. That is the defining cruelty of glaucoma, a group of eye conditions that systematically destroys the optic nerve and stands as the leading cause of irreversible blindness in the United States. According to the Glaucoma Research Foundation, more than three million Americans currently live with the disease, and projections suggest that number will climb to 4.2 million by 2030. Perhaps most alarming: roughly half of those affected remain entirely undiagnosed.

What Exactly Is Glaucoma?
Glaucoma is not a single disease but rather a family of conditions united by one outcome: progressive damage to the optic nerve, the critical bundle of fibers that transmits visual information from the eye to the brain. In the most common form — primary open-angle glaucoma — the eye's internal drainage system becomes less efficient over time, causing intraocular pressure (IOP) to rise. That elevated pressure gradually compresses and erodes optic nerve fibers.
What makes open-angle glaucoma so insidious is that it produces no pain. There is no redness, no visual distortion, no early warning sign that most patients would notice or report to a physician. Vision loss begins at the edges of the visual field, where the brain is adept at filling in perceived gaps. By the time a person recognizes that something is wrong, they may have already lost 30 to 40 percent of their optic nerve function — damage that cannot be reversed.
A less common but more dramatic variant, acute angle-closure glaucoma, does present suddenly with severe eye pain, nausea, blurred vision, and halos around lights. This constitutes a medical emergency requiring immediate treatment. However, the silent open-angle form accounts for the vast majority of cases in the United States.
Who Is Most at Risk?
While glaucoma can affect anyone, certain populations carry a substantially elevated risk. Recognizing whether you fall into one or more of these categories should inform how proactively you pursue screening.
Age is among the most significant factors. Adults over 60 face a markedly higher probability of developing glaucoma, and the risk escalates further after age 70. For African Americans, elevated risk begins earlier — studies indicate the condition is three to four times more prevalent in Black Americans than in white Americans of comparable age, and it tends to progress more aggressively.
Family history also plays a critical role. Having a parent or sibling diagnosed with glaucoma increases your own likelihood of developing it by four to nine times, according to the American Academy of Ophthalmology. The disease has a well-documented genetic component, and clinicians consistently identify familial clustering.
Additional risk factors include:
- Elevated intraocular pressure — the most well-established modifiable risk factor
- Severe nearsightedness (myopia) — high myopia is associated with structural changes in the optic nerve that increase vulnerability
- Prolonged corticosteroid use — including steroid eye drops, inhalers, and oral medications
- History of eye injury or surgery
- Conditions such as diabetes, hypertension, and sleep apnea — all linked to increased glaucoma risk through vascular mechanisms
- Hispanic/Latino heritage — this population experiences glaucoma prevalence rates approaching those of African Americans
It is worth emphasizing that elevated IOP alone does not confirm glaucoma, and some individuals develop the disease with normal pressure readings — a variant known as normal-tension glaucoma. This is one reason why comprehensive evaluation extends well beyond a single pressure measurement.
The Diagnostic Landscape: What a Thorough Screening Involves
A pressure check alone — the familiar puff-of-air test — is insufficient for detecting glaucoma with confidence. A complete glaucoma evaluation incorporates several complementary assessments.
Tonometry measures intraocular pressure and remains a standard component of any eye exam. However, as noted above, it tells only part of the story.
Ophthalmoscopy allows the clinician to directly examine the optic nerve head, assessing for characteristic signs of damage including an enlarged cup-to-disc ratio.
Visual field testing (perimetry) maps the patient's full field of vision, identifying areas of loss that may not yet be subjectively apparent.
Optical coherence tomography (OCT) represents one of the most significant advances in glaucoma detection. This imaging technology produces high-resolution cross-sectional scans of the optic nerve and retinal nerve fiber layer, enabling clinicians to identify structural thinning before functional vision loss occurs. Many eye care practices across the United States now integrate OCT as a standard diagnostic tool.
Pachymetry, which measures corneal thickness, provides important context for interpreting IOP readings, as thinner corneas may cause tonometers to underestimate true pressure.
The American Academy of Ophthalmology recommends that adults with no known risk factors receive a comprehensive eye exam by age 40 and every one to two years thereafter. For individuals in high-risk groups, earlier and more frequent evaluations are strongly advised.
Treatment: Managing a Condition That Cannot Be Cured
There is currently no cure for glaucoma, and lost vision cannot be restored. Treatment is therefore focused entirely on halting or slowing progression by reducing intraocular pressure.
Prescription eye drops remain the first-line intervention for most patients, working either by decreasing fluid production within the eye or by improving its drainage. Adherence to a consistent daily regimen is essential — irregular use significantly undermines effectiveness.
For patients whose pressure is inadequately controlled through medication, laser procedures such as selective laser trabeculoplasty (SLT) offer a minimally invasive option that improves drainage with a strong safety profile. Surgical interventions, including trabeculectomy and minimally invasive glaucoma surgery (MIGS), are reserved for more advanced or refractory cases.
Regardless of treatment pathway, regular monitoring is non-negotiable. Glaucoma management is a lifelong commitment, not a one-time intervention.
