Every year, Americans fill billions of prescriptions and purchase hundreds of millions of packages of over-the-counter medications. The warnings printed on those labels — drowsiness, nausea, stomach upset — are familiar enough that most people skim past them. But buried deeper in the fine print, or sometimes absent entirely from consumer-facing materials, is a category of side effects that rarely gets the attention it deserves: ocular toxicity.

The relationship between systemic medications and eye health is well-documented in clinical literature, yet it remains one of the least-discussed topics in routine patient care. Physicians managing blood pressure or allergies are not always focused on the retina. Eye care professionals, meanwhile, may not have a complete picture of a patient's medication history. The result is a significant gap — one that can allow vision-threatening side effects to progress unnoticed for months or even years.

Antihistamines: The Allergy Relief That Dries Out Your Eyes

Over-the-counter antihistamines — including diphenhydramine (Benadryl), cetirizine (Zyrtec), and loratadine (Claritin) — are among the most widely used medications in the United States. Millions of Americans reach for them during allergy season, or even nightly as a sleep aid, without a second thought.

What many do not realize is that antihistamines have an anticholinergic effect, meaning they block the neurotransmitter acetylcholine throughout the body. In the eyes, this translates to a measurable reduction in tear production. The result is dry eye syndrome — a condition characterized by irritation, burning, blurred vision, and increased sensitivity to light.

Beyond dryness, antihistamines can also cause pupil dilation. For individuals with a narrow anterior chamber angle in the eye — a structural variation they may not know they have — this dilation can trigger a dangerous spike in intraocular pressure, a condition known as acute angle-closure glaucoma. This is a medical emergency. Anyone with a family history of glaucoma or who has been told they have narrow angles should consult an eye care professional before relying on antihistamines regularly.

Corticosteroids: Powerful Anti-Inflammatories With a Hidden Cost

Corticosteroids, whether taken orally (such as prednisone), inhaled for asthma, applied topically to the skin, or used as eye drops, are among the most effective anti-inflammatory agents in medicine. They are also among the most consequential for long-term eye health.

Prolonged use of corticosteroids is strongly associated with two major ocular complications. The first is posterior subcapsular cataracts — a specific type of cataract that forms at the back of the eye's lens and can cause significant glare and difficulty with near vision. The second is steroid-induced glaucoma, in which the medication raises intraocular pressure in a subset of patients who are genetically predisposed to this response.

Patients taking corticosteroids for conditions such as rheumatoid arthritis, lupus, asthma, or inflammatory bowel disease are often under the care of specialists who may not routinely order eye exams. If you take any form of corticosteroid on an ongoing basis, it is worth discussing regular intraocular pressure monitoring and lens evaluations with an eye care provider.

Hydroxychloroquine: A Lupus and Arthritis Drug With Retinal Consequences

Hydroxychloroquine (Plaquenil) is commonly prescribed for lupus and rheumatoid arthritis, and it is generally considered one of the safer long-term medications in those fields. However, it carries a well-established risk of a condition called hydroxychloroquine retinopathy.

With prolonged use — typically five years or more, or at higher doses — the drug can accumulate in retinal cells and cause irreversible damage to the macula, the central portion of the retina responsible for sharp, detailed vision. Early changes are often asymptomatic, which is precisely why the American Academy of Ophthalmology recommends baseline eye exams before starting the medication, followed by annual screenings after five years of use.

If you are taking hydroxychloroquine and have not had a dilated eye exam recently, this is a conversation worth initiating with your prescribing physician.

Statins and Blood Pressure Medications: Subtler Effects Worth Knowing

Statins, the cholesterol-lowering drugs taken by tens of millions of Americans, have been associated in some studies with a modest increase in the risk of cataracts, though the evidence remains debated. More clearly documented is a rare side effect known as diplopia — double vision — associated with certain statin formulations in a small subset of patients.

Some beta-blockers used for blood pressure management can reduce tear secretion, contributing to dry eye symptoms. Amiodarone, a cardiac medication, can cause corneal deposits that create halos around lights, though these are generally reversible upon discontinuation. Certain erectile dysfunction medications, including sildenafil (Viagra), have been linked in rare cases to non-arteritic anterior ischemic optic neuropathy (NAION), a sudden loss of blood flow to the optic nerve.

What You Can Do Right Now

The goal here is not to alarm anyone into stopping necessary medications. The vast majority of people taking these drugs will not experience significant ocular side effects. Rather, the aim is to encourage a more proactive posture.

Compile a complete medication list before your next eye exam. Include prescriptions, over-the-counter drugs, supplements, and inhalers. Your eye care provider can cross-reference this list against known ocular side effect profiles.

Ask specifically about monitoring. If you take hydroxychloroquine, corticosteroids, or any medication on this list long-term, ask your doctor what screening schedule is appropriate for your situation.

Do not dismiss new visual symptoms. Blurred vision, halos, color changes, or persistent dryness that coincides with starting a new medication warrants a call to both your prescribing physician and your eye care provider.

Consider lubricating eye drops for drug-induced dryness. If antihistamines or other anticholinergic medications are contributing to dry eye, preservative-free artificial tears can provide meaningful relief while you and your doctor explore longer-term solutions.
